Brief Description

JOB SUMMARY: The Nurse I or II works in the WMS (Detox) setting as a part of a multidisciplinary team to provide medical detoxification and stabilization services for detox and residential clients in compliance with all BestCare policies and procedures, best nursing care practices, and all local, state, and federal regulations. ESSENTIAL FUNCTIONS:

Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following detox and nursing division’s philosophies and standards of care set by the state board of nursing, state nurse practice act, and OAR’s for detox programs;
Resolves patient problems and needs by utilizing multidisciplinary team strategies;
Ensures a safe and clean working environment by complying with procedures, rules, and regulations;
Protects patients and employees by adhering to infection-control policies and procedures, medication administration and storage procedures, and controlled substance regulations;
Documents client progress in electronic health record (EHR) system to maintain a pertinent record of client activity according to established methods and procedures;
Participates in daily staff organizational meetings and treatment planning review meetings;
Takes a lead role or participates in providing medication administration and medical services;
Other related duties as assigned.

Brief Description

JOB SUMMARY

The Licensed Medical Provider in this role works in an acute withdrawal management (or detox) setting as well as in a residential treatment environment, caring for patients suffering from substance use disorders. The provider in this role works as a part of a larger addiction medicine team, including experienced physicians, advance practice providers, PMHNPs, as well as RN’s, LPN’s, and substance use counselors. The provider performs admission physicals, formulating and documenting a patient-centered plan of care for both management of withdrawal and initiation of maintenance medications to manage substance use disorders. The provider also addresses other medical issues that arise in detox and residential clients as needed; participates in coordination of care with other team members internally, as well as with external providers including primary care providers, specialists, and the Emergency Department, when indicated. This role will spend an average of approximately 30-40 hours per week, in a 3-4 day work week. An average day consists of 4-8 hours of direct patient care and coordination, with 24 hour on-call access to provide admission orders for evening intakes and to triage occasional issues as they arise.

ESSENTIAL FUNCTIONS

Screens referrals for potential admission to addiction medicine services;
Conducts histories and physicals on patients within 24 hours of inpatient admission and within 72 hours of outpatient visit, and documents assessment and plan in electronic medical record (EHR);
Reviews nursing assessment done at inpatient admission and issues standing orders;
Issues any additional procedure orders for continuation of medications or initiation of medications needed to address ongoing medical conditions as well as lab orders;
Orders necessary labs and review results with patients;
Orders and interprets urine toxicology screens, and accesses the Oregon Prescription Drug Monitoring Database when necessary;
Provides counseling and education on medication options for the treatment of various substance use disorders;
Provides education regarding options for and rationale behind healthy SUD recovery and medical concepts related to addiction;
Formulates and initiates treatment plan with patient’s informed consent and participation; Titrates or adjusts medications as needed;
In the inpatient environment, reviews CIWA/COWS/Vitals results (conducted by Nursing staff) to determine if patient’s withdrawal symptoms have stabilized sufficiently for discharge on a day-to-day basis;
Administers medications for SUD treatment via subcutaneous/intramuscular injections, using standard practices including clean technique (training provided);
Uses motivational interviewing and active listening to support clients recovering from drug/alcohol dependence;
Addresses other general health concerns as they arise (to include contraceptive counseling, STI counseling and treatment, routine psychiatric care, emergent/temporary management of issues such as hypertension and diabetes, and treatment of chronic viral infections) and documents assessments and medical care in EHR, or refers to primary care, specialty care, or the emergency room as needed.
Completes and signs medical orders (meds, labs, etc.) in a timely fashion;
Reviews outside health records when necessary;
Documents diagnosis, level of service, and procedure codes using EMR;
Provides after hours support to nursing, counseling, and respite staff (when on-call) to address general medical concerns and questions;
Coordinates care both internally (with outpatient counseling, Addiction Medicine clinic, and residential treatment team members) as well as externally (with community PCPs, ER staff, and external AMC clinics) as needed;
Assures quality of care by adhering to therapeutic standards; measures health outcomes against patient care goals and standards, making or recommending necessary adjustments; follows detox and Medical Board philosophies and standards of care set by the state Medical Board, and OAR’s for detox programs;
Performs other related duties as assigned and approved by the CMO and BestCare Administration.

JOB SUMMARY: As part of the Program’s Treatment Team, the Substance Use Disorder (SUD) Counselor provides individual and group counseling services to patients participating in the Program as well as to clients and to family members on theories and treatment of substance use disorders using a patient-centered approach. The SUD Counselor also functions as community liaison for a patient-centered and harm-reduction approach to substance use disorder treatment.

ESSENTIAL FUNCTIONS:

Works closely with the Treatment Team with assigned caseload of clients enrolled in the Program;
Completes intakes, assessments and develops treatment plans as appropriate using American Society of Addiction Medicine (ASAM) criteria to assess for appropriate levels of patient care (LOC) and integration with Mental Health and SUD programs;
Collaborates with clients on completion of individualized treatment plans that contain goals and objectives that are measurable and are based on assessed needs and problems;
Provides treatment through a variety of modalities including individual sessions, group counseling, and family counseling, as well as case management services, ensuring individualized, quality care;
Conducts counseling sessions for individuals, groups, and persons diagnosed with substance use disorders working collaboratively with treatment team members and other agencies and applying principles, procedures, techniques, trends, and literature of individual and group counseling;
Provides crisis intervention as needed;
Assesses ongoing needs of individuals, update assessments and recommendations as needed, and make appropriate referrals for additional services as needed;
Works effectively with all recovery resource providers and all agencies in the community to provide seamless continuum of care for patients with substance use disorders;
Works effectively and respectfully with client and family populations with diverse backgrounds and needs;
Leads psycho-educational, skill development, relapse prevention, process, gender-specific, and medication supported recovery groups as assigned;
Provides information about the Program to individuals, the public and an outreach to community groups;
Collaborates with Certified Recovery Mentors on patient care and provides support and direction as needed;
Collaborate with clients on discharge and aftercare planning;
Maintains compliance with all state and federal laws and regulations, as they pertain to position including HIPAA, scope of practice, accreditation standards, OHA and OSHA;
Works closely with referral sources, keeping them informed on the progress of the client, as needed and appropriate for client confidentiality;

Brief Description

JOB SUMMARY

The Licensed Medical Provider in this role works in an acute withdrawal management (or detox) setting as well as in a residential treatment environment, caring for patients suffering from substance use disorders. The provider in this role works as a part of a larger addiction medicine team, including experienced physicians, advance practice providers, PMHNPs, as well as RN’s, LPN’s, and substance use counselors. The provider performs admission physicals, formulating and documenting a patient-centered plan of care for both management of withdrawal and initiation of maintenance medications to manage substance use disorders. The provider also addresses other medical issues that arise in detox and residential clients as needed; participates in coordination of care with other team members internally, as well as with external providers including primary care providers, specialists, and the Emergency Department, when indicated. This role will spend an average of approximately 30-40 hours per week, in a 3-4 day work week. An average day consists of 4-8 hours of direct patient care and coordination, with 24 hour on-call access to provide admission orders for evening intakes and to triage occasional issues as they arise.

ESSENTIAL FUNCTIONS

Screens referrals for potential admission to addiction medicine services;
Conducts histories and physicals on patients within 24 hours of inpatient admission and within 72 hours of outpatient visit, and documents assessment and plan in electronic medical record (EHR);
Reviews nursing assessment done at inpatient admission and issues standing orders;
Issues any additional procedure orders for continuation of medications or initiation of medications needed to address ongoing medical conditions as well as lab orders;
Orders necessary labs and review results with patients;
Orders and interprets urine toxicology screens, and accesses the Oregon Prescription Drug Monitoring Database when necessary;
Provides counseling and education on medication options for the treatment of various substance use disorders;
Provides education regarding options for and rationale behind healthy SUD recovery and medical concepts related to addiction;
Formulates and initiates treatment plan with patient’s informed consent and participation; Titrates or adjusts medications as needed;
In the inpatient environment, reviews CIWA/COWS/Vitals results (conducted by Nursing staff) to determine if patient’s withdrawal symptoms have stabilized sufficiently for discharge on a day-to-day basis;
Administers medications for SUD treatment via subcutaneous/intramuscular injections, using standard practices including clean technique (training provided);
Uses motivational interviewing and active listening to support clients recovering from drug/alcohol dependence;
Addresses other general health concerns as they arise (to include contraceptive counseling, STI counseling and treatment, routine psychiatric care, emergent/temporary management of issues such as hypertension and diabetes, and treatment of chronic viral infections) and documents assessments and medical care in EHR, or refers to primary care, specialty care, or the emergency room as needed.
Completes and signs medical orders (meds, labs, etc.) in a timely fashion;
Reviews outside health records when necessary;
Documents diagnosis, level of service, and procedure codes using EMR;
Provides after hours support to nursing, counseling, and respite staff (when on-call) to address general medical concerns and questions;
Coordinates care both internally (with outpatient counseling, Addiction Medicine clinic, and residential treatment team members) as well as externally (with community PCPs, ER staff, and external AMC clinics) as needed;
Assures quality of care by adhering to therapeutic standards; measures health outcomes against patient care goals and standards, making or recommending necessary adjustments; follows detox and Medical Board philosophies and standards of care set by the state Medical Board, and OAR’s for detox programs;
Performs other related duties as assigned and approved by the CMO and BestCare Administration.

Brief Description

JOB SUMMARY: The Certified Recovery Mentor (CRM) provides support and directs services to people from the Residential, Detox and Outpatient Programs. CRMs will also assist people from other programs on a limited basis as needed, and provides regular support to clients through direct interactions, advocacy, and provision of transportation and referrals to community-based resources, as well as facilitation of classes, support groups, and group activities.

ESSENTIAL FUNCTIONS:

Participates and provides recovery coaching to people involved with BestCare Treatment Services through the following methods:
Telephone contact with clients
Provides client transportation
Provides recovery coaching for clients during activities
Makes home visits on a limited basis
Supervises sober housing as needed
Assists prep and clean-up for evening groups;
Provides individual, couples, and family support;
Attends program and all staff meetings as well as weekly supervision meetings;
Provides direct support to detoxing clients as needed;
Provides direct support to clients as they transition from detox to outpatient services;
Works with the Supervisor to design and implement education, referral, and parent components of the program;
Helps to facilitate classes and support groups along with group activities to ensure that they are sufficiently supervised and run smoothly;
Maintains case notes of client contact;
Completes attendance sheets, required report forms, and report client participation to Supervisor;
Maintains a personal recovery program, if appropriate, as well as models recovery-appropriate behavior;

JOB SUMMARY:

The Nurse I or II works in the WMS (Detox) setting as a part of a multidisciplinary team to provide medical detoxification and stabilization services for detox and residential clients in compliance with all BestCare policies and procedures, best nursing care practices, and all local, state, and federal regulations. Responsibilities include provision of medication management, taking patient vitals, maintenance of pharmacy/medical supplies, coordination of care with primary care providers for our mental health clientele, and advocacy for excellent patient care to achieve the best patient outcomes.

ESSENTIAL FUNCTIONS:

Takes vital signs, such as blood pressure, temperature and CWAA scores;

Gives injections of medications; immunizations as prescribed by Program and Medical Director,

Resolves patient problems and needs by utilizing multidisciplinary team strategies;

Protects patients and employees by adhering to infection-control policies and procedures, medication administration and storage procedures, and controlled substance regulations;
Documents client progress in electronic health record (EHR) system to maintain a pertinent record of client activity according to established methods and procedures;

JOB SUMMARY: The Mental Health Clinician I provides individual and group services as a part of a multi-disciplinary team of behavioral health professionals to provide or coordinate rehabilitation and community support services for clients who are seeking treatment for mental health and substance use disorders.

ESSENTIAL FUNCTIONS:

Complete intakes, assessments, and individualized treatment plans as appropriate;
Works closely with other staff as a member of a multi-disciplinary treatment team to achieve the best outcomes for the clients;
Identifies precipitating events, gathers histories of mental, emotional and physical disabilities, alcohol and drug use and substance abuse disorders, past mental health services and criminal justice contacts; assesses family, social, and work relationships; conducts mental status assessments; documents a multiaxial DSM diagnosis; writes and implements or supervises implementation of a treatment plan; conduct and documents mental health assessments; and provide mental health treatment and rehabilitative services within the scope of his or her practice;
Develops and maintains strong collaborative relationships with a wide range of community partners, including, but not limited to: County DHS and Health Departments, Adult Community Justice, Public Health, and other programs and departments;
Provides individual, group, family counseling, and case management, working collaboratively with treatment team members and other agencies;
Complete clinical chart reviews, as assigned;
Maintains accurate documentation of all services in client files in EHR within agency stated timelines;
Leads psycho-educational, skill development, process, gender-specific, and family groups as assigned;
Works closely with referral sources, keeping them informed on the progress of the clients, as needed and appropriate to client confidentiality per HIPAA and 42 CFR and Oregon statutes;
Models appropriate counselor behavior to other staff; helps train and orient new staff to the program as necessary;
Completes training/certification required to perform mental health crisis work and possibly be part of the MH crisis rotation;
